Scleral Buckling & Retinopexy

Retina eye surgery is done in order to repair a detached retina.

It's known as scleral buckling.

Retina lines the back inside surface of the eye, and converts the optical image into nerve impulses that travel through the optical nerve to the brain where they turn back into the optical image.

The pressure of the gel and fluid, that fills the eye, keep the retina held in place against the wall of the eye.

A hole in the retina deflates this pressure resulting in a retinal detachment.

Victims to myopia and diabetic retinopathy are more prone to having such holes in their retina.

As the retina gets detached, it stops getting its proper blood supply from vessels in the wall of the eye and its cells start dying.

Hence, retina eye surgery becomes an emergency to be performed as early as possible.

Retinal detachment causes one to keep seeing flashing lights in one's field of vision.

If the holes are detected getting formed in the retina but the detachment has not yet taken place, they can be sealed with a laser through a non-invasive surgery.

If the detachment has recently started, pneumatic retinopexy is done. Injecting a gas bubble into the eye, with the patient positioned so that it floats up against the hole in the retina and pushes it back into place, does the job. Then the hole can be sealed with a laser.

For more advanced detachments, following surgical procedures are adopted:

  • Scleral Buckling: The wall of the eye is bent-in to meet the hole in the retina.

  • Vitrectomy: Very small instruments inside the eye are used to remove a scar pulling the retina forward.

  • Combination: If a detachment has gone very complex, the two procedures in conjunction can be used during the same surgical operation.

Once the retina gets detached, it doesn't repair itself on its own.

If left un-repaired, it very fast leads to serious vision loss and even to total blindness!

Also, if the detachment has not yet affected the central vision area, i.e., the macula; no time is there to be wasted and immediately the surgery should be taken up so that the macula could be saved from getting affected.

But if the macula has already detached, prognosis is not very good; and hence it is no more an emergency situation.

The patient should ready her/himself to face the following risks involved in the retina eye surgery:

  • Reactions to Medications

  • Problem in Breathing

  • Bleeding

  • Infection

  • Failure in Retina Getting Attached

The prognosis of the retina eye surgery depends on:

  • the number of the holes,

  • the size of the holes,

  • and the presence of scar tissues on the retina.

The quality of the vision recovered depends on:

  • Long Duration Involvement of Macula: not good!

  • Short Duration Involvement of Macula: so-so!

  • No Involvement of Macula: very good!

With a retina eye surgery of any kind whether non-invasive with laser, pneumatic retinopexy, scleral buckling, or vitrectomy; no overnight stay in the hospital is usually needed but the activities get limited for a period of time.

But shouldn't we give it a thought as to why, after all, does the retina get detached; or develop holes or scars in it in the first place?

Is it the normal wear and tear of the retina?

Certainly not!

It has to be abnormal.

There must be some abnormal mechanical forces acting upon it making its wear and tear possible.

Why do we let them act?

Or do we make them act!?

Of course, through our musculature keeping our eyes under pressure of mechanical tension on them!

And they continuously remain under pressure because we have gone habitual of keeping this mechanical tension exerted upon them as a way of life, all the 24 hours, 7 days, and 52 weeks of the year!

They never go relaxed!

Wear and tear has to be there.

Why do we go habitual of keeping our eyes under mechanical tension?

Because we have gone habitual of keeping our body posture under mechanical tension!

The two do have a connection.
